    Tangible, Aligned Outcomes
    Measure what has actually changed

    We return to the outcome established at the beginning and assess what has actually changed. Has the issue shifted? Is the outcome being achieved? And, importantly, has what was really driving the issue been addressed rather than the symptom temporarily improving?

    This closes the loop. We use what has changed, and what has not, to determine whether further attention is needed and where it should be focused.

    Measurable Progress: Assess whether the outcome is being achieved and what is holding in practice.
